If somebody is "out of it," this means that they are not thinking clearly, perhaps because of drugs or alcohol, or just from being very tired. I'm in over my head. If somebody is "in over their head," they are involved in something that is too difficult or complex for them to understand or do well in. Like a rash? A "rash" is reddish outbreak on the skin.
